Safety Notifications

This form is to be used to notify the Risk Management Department about possible hazards throughout the campus area. These hazards may include inadequate or malfunctioning lights, uneven sidewalks, or missing fire extinguishers. This form may also be used for suggesting ways to improve campus safety. Please fill out this form on your computer or by hand. A hard copy of this form may be submitted to the Risk Management office located in the Luther tunnel 116 or it may be e-mailed as an attachment to smulso@csp.edu.
