Confidentiality

All medical records and information obtained in the Health Center are confidential. Information will not be released to anyone (faculty, staff, parents, doctors, etc) without the students written consent.

If parents request information they will be told to contact the student for the information. Only in life threatening emergencies will any information be given to parents.

Students should discuss with professors any health problem or treatment that may influence academic performance. If the student requests the health center to verify an illness or appointment to an instructor, that will be done, but no information will be given concerning the reason for the visit unless requested in writing by the student.

Students may obtain a copy of medical records or request it be sent to another institution/agency by completing a Release of Medical Information form.
