Alumni and Disability Services

 

Receiving Copies of Your Documentation from Disability Services:

  1. We cannot, under any circumstances, release information to another outside agency without written authorization from the student that information can be released.  In addition, disability services will not release accommodation plans to outside agencies without the express written consent of the student. This is because outside agencies do not have a ?need to know? in relation to our programs and laws that pertain to documentation.

    If you need documentation, please either obtain your own copy from our files prior to leaving school, or be sure to sign a waiver that will allow release of your information to the appropriate parties. 

    Due to law's pertaining to confidentiality, we can make no exceptions in this policy.

Documentation:

  1. Documentation on a student is kept in locked storage files for 5 years past the last date of attendance.  This information is then shredded.  Students are welcome to request shredding of their files prior to this timeline.
  2. Documentation Reviews:  Once a student has left Concordia: Disability Services cannot write accommodation plans for students who have left Concordia and not previously received services through our office. One a student leaves Concordia, we also cannot review new documentation and make recommendations.  This should be done by a professional who has a working relationship with the student and knowledge of their current level of functioning.

Letters of Recommendation:

Staff in Disability Services can choose to write letters of recommendation for students who have had a positive working relationship with a staff member.  Each individual staff member makes will use their best judgment in determining these recommendations and reserves the right to decline a request. 

Please remember that true letters of recommendation should be written by individuals who have a current and positive working relationship with the student. Letters that confirm the existence of disability or accommodation can be written but do not constitute support or rejection of the student's application.

For letters of confirmation of disability or recommendation require at least 5 days notice.
