Client Information Sheet
Each student is required to complete this form prior to their first appointment for each academic year (even if they have been seen here for counseling in previous years).  Print the form, fill it out, and bring it to your initial appointment.  You will be asked to complete some additional brief paperwork prior to your first appointment.  If you need this additional paperwork in an alternative format, please contact us before your initial appointment so that we can determine which forms to give to you.

Referral Sheet for Student Services
If you would like to refer a student to Counseling Services or other campus offices, please use this form to assist your referral.

Evaluation of Counseling Services
If you have been seen in counseling, please take a moment to print off the following anonymous evaluation of your experience.  You can mail it to our office via campus mail (Counseling Services, DH 310).  We typically ask students to fill out an evaluation at the end of each semester but you may complete one at any time.

Evaluation of Other Services Received
If you have received other non-counseling related services provided by our office (consultation, outreach, training, etc.) please take a moment to print off this anonymous evaluation, complete it, and mail it to our office via campus mail (Counseling Services, DH 310).

Client Bill of Rights
This is posted in the Counseling Services main office and requests can be made for hard copies of this document if you do not wish to view it or print it off from this link.  This document explains your rights as clients receiving counseling services.
